**If this is an emergency request, or you have a hold and need the FI complete within the next 48 hours, please call our office and ask for the Forensic Interviewer on call to schedule.
All incidents must be reported to law enforcement or child welfare before a forensic interview can be scheduled. Only law enforcement or child welfare workers may complete this form.
